About this test
Regulated testing leaves no room for procedural error. Our collectors are trained and qualified under 49 CFR Part 40 and use the federal Custody and Control Form for every DOT collection. We handle pre-employment, random, reasonable-suspicion, post-accident, return-to-duty and follow-up testing, and we coordinate directly with your Designated Employer Representative and Medical Review Officer. Results are reported to the FMCSA Clearinghouse where applicable.

- A valid government-issued photo ID (required).
- Any employer authorization form, agency referral or court order.
- A list of prescription medications, in case the MRO needs to contact you.
